Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications, treatments, and skilled nursing procedures (e.g., wound care, IV therapy).
Monitor patient conditions, document care, and communicate changes to physicians and families.
Educate patients and caregivers on disease management, medications, and self-care techniques.
Ensure compliance with clinical protocols, infection control, and safety standards.
Current, unrestricted RN license in the state of Kentucky.
Minimum of 1 year of clinical nursing experience (home health preferred).
Strong critical thinking, communication, and documentation skills.
Ability to work independently and manage multiple patients.
CPR certification and up-to-date immunizations.
Provide direct nursing care under the supervision of an RN or physician.
Administer prescribed medications and treatments, including wound care and injections.
Monitor vital signs, patient responses, and report changes promptly.
Assist with daily living activities and provide patient and family education as directed.
Maintain accurate clinical documentation per agency policies.
Current, unrestricted LPN license in the state of Kentucky.
At least 1 year of nursing experience (home health or long-term care preferred).
Strong patient care, time management, and interpersonal skills.
Ability to follow care plans and work collaboratively with healthcare teams.
CPR certification and required immunizations.
